Grooming Waiver
At Danmar Grooming, your pet is important to us. We want to assure you that every effort will be taken to make sure your pet's bathing experience is pleasant. Occasionally, grooming can expose a hidden medical problem or aggravate a current condition. This can occur during or after grooming
Because a pet could be severely tangled or matted, there is a greater risk of injury, stress and trauma. All precautions will be taken. However, problems occasionally arise, during or after grooming: such as nicks, clipper irritation and mental/physical stress. I understand that if my pet is matted, many pet has a greater chance of injury during grooming. I will not hold this grooming establishment responsible for accident or injury to my pet.
In the best interest of your pet, we request permission to obtain immediate veterinary treatment, at your expense, should it become necessary. In the even that I am not present, I hereby grant permission to Danmar Grooming to obtain veterinary treatment for my pet at my expense. The undersigned further grants full permission to Danmar Grooming to use any photographs or videotapes from grooming for any purpose. Additionally, I have notified Danmar Grooming of any pre-existing medical conditions my pet may have had prior to receiving grooming services. I hereby agree that my pet is up to date on RABIES and all other necessary vaccinations.
